Find it in a blue-spiked chest in the Volcanic Biome; carry it to the volcano altar for a two-charge sacrifice.

Using the altar
Take the totem to the platform inside the volcano and throw it into the lava to advance the altar by two charges. The altar pays out on its charge cycle, eventually offering infernal weapons as well as Cultist Gems and a Gem of the Forest fragment.
