sanity. The Surgeon is one of the playable classes. They can be unlocked for 2,500 Animal Coins.
The Surgeon is an obtainable class that can be purchased for 2500 Animal CoinsAnimal Coins in the Lobby Classes shop. It is currently the most expensive class in the game being sold with the Animal Coin currency. Its perks allow the player to recover Sanity and gain a short speed boost after Treating a Patient, similar to using Run Fast Cola. At level 1, treating a patient recovers 1 Sanity and gives the player a 4 seconds speed boost. At level 2, treating a patient will give the player a longer, 8 seconds speed boost. At level 3, the player recovers 2 Sanity after treating a patient and starts out with 10 bonus sanity. The Surgeon starts out with $35.

Appearance
The Surgeon is a dark orange moose-like creature with lighter orange patterns on their face. The surgeon wears white medical scrubs under a turquoise consultation coat. Wrapped around their neck is a black stethoscope. On their head rests a blue bouffant cap and a head mirror with a brown band. Their face features slit eyes with white irises, and their mouth is covered by a blue surgical mask. They also have two antlers that are similar to the color of the patterns on their face on either side of their head.
Strategies
Similar to Doctor, the Surgeon is a class focused on treating patients, staying around the Medical Wing and the Emergency Wing to treat them, with an additional speed boost after healing a patient. The Surgeon is statistically superior to the Doctor class, only having lower bonus sanity, negligible after taking Coffees. Its speed boost perk allow the player to treat multiple patients quicker, making them the better option in later Shifts, especially during Events and Emergencies. For example, during an Ambulance Event in later shifts, more patients in critical condition will spawn in, requiring the player to be quick to treat patients on death timers. Like the Doctor class, it is recommended to focus solely on treating patients when playing in a multiplayer run.
Known Bugs
Finishing a surgery does not give the player speed boost.
Patch history
The Surgeon used to share the same model with Dr. Harlow's first model, until it was changed in the Harlow's Favorite Update on July 10, 2026. The initial reworked model for Surgeon originally featured reddish fur and a thicker antlers model. The fur was later changed to a brownish tone and the antlers were changed. The first model is still available as the Classic skin, priced at 250 Animal CoinsAnimal Coins.
