Twisted Sprout has an appearance similar to his Toon counterpart, but noticeably taller and almost completely consumed by Ichor, along with having a more desaturated color palette. When Twisted Sprout spots a Toon, he will pause his movement, spawning a puddle of Ichor at that Toon's position containing a tendril that, shortly after being summoned, will attack anyone who comes too close to it. This ability has a cooldown of 10 seconds, and the tendril disappears after 15 seconds, meaning that the maximum amount of tendrils that can be on the floor at the same time is 2, though there's no limit to how many Twisted Sprout can spawn during a chase. While wandering around, Twisted Sprout will occasionally try to follow if he spots him on the floor.

- When detecting a Toon, Twisted Sprout will leave a deadly tendril under the detected Toon, attacking anyone that steps on it for too long. Has a cooldown of 10. If Twisted Cosmo is on the floor as well, Twisted Sprout will occasionally follow him. He will also sometimes stop to look at posters of his Toon counterpart.

His legs, mouth, and left arm are covered in Ichor. His left arm is much larger than the other and has what is presumed to be a large claw made of Ichor. Additionally, the leaves of his hair are noticeably longer, pointier, and duller than his Toon counterpart's, and his bangs are covering up his left eye. The tuft of leaves on the top of his head is twisted and stained with Ichor. A few extra leaves appear to be growing from behind his neck. His scarf, which is still white with thin pink stripes, is longer, has a pointy, Ichor-covered end, and seems to be sentient. His visible eye is red with a small pupil, like most of the Twisteds, and appears to be crying Ichor. His blush is smeared. Upon spotting a Toon, his expression becomes sadder.
Strategies
As Twisted Sprout is capable of summoning non-movable tendrils, vacate any Machines before you are in his detection range, to reduce/eliminate time taken for tendril dissipation. When you are planning to distract him, avoid cramped rooms with a lack of exits, as it will decrease your chance of survival. Having many exits reduces the amount blocked by his tendrils and makes escapes easier. While distracting him in open areas, hugging one side of either the island or outer walls reduces the area the tendril covers (unless other aggressive Twisteds are chasing you). Also, if done quickly and across the edges, it is possible to run through his tendrils without taking damage. His short cooldown and tendrils should be noted for slower or low-Stamina Toons, like, or, as running while he is spawning tendrils is a good way to lose him, so preserve Stamina for the occasion. Since his abilities are physical, try to keep him away from so their attack radiuses don't intersect, as this could cause problems. Similarly to them, his tendril attacks can be avoided even if you step into the attack range circle if you're quick at leaving it. Unlike the other Twisteds who have almost the same or higher chasing speed as/than regular Twisteds, Twisted Sprout is the slowest Main Character Twisted, meaning that it's possible for 4 Movement Speed Toons or a ≤2 to outrun him easily and even out-walk him without the use of Movement Speed Trinkets, albeit by a tiny margin. Do note that Panic Mode significantly increases the speed of Twisteds, letting him catch up to you easier.
Patch history
Twisted Sprout is the second Twisted with a potential ranged attack, with the first being, and, the third being, and the fourth being. Additionally, Twisted Sprout is the first and currently only ranged Twisted to be a and whose ranged attack is not avoided by hiding being an obstacle. This makes him the first Twisted to have an ability that summons a trap, with the second being, and the third being Twisted Blot. Twisted Sprout and Twisted Blot's are quite similar, being 2 black traps that sometimes appear and disappear and damage Toons if they stay near/in them for too long. Out of these, Twisted Sprout, Twisted Razzle & Dazzle, Twisted Rodger, and Twisted Blot's ranged attacks are the only ones that can't be blocked by. Twisted Sprout and have matching Ichor patterns, as confirmed on the Discord by Qwelver. Twisted Sprout has Ichor on his left eye and arm, and Twisted Cosmo has Ichor on his right eye and arm. This reflects their Toon counterparts' relation with each other. Twisted Sprout is the tallest Twisted in the game (not counting or 's T-pose model), with the second being. The sounds used for Twisted Sprout's idling growls originate from Half Life 2's Poison Zombie, being "pz_breathe_loop1" and "pz_alert2". Twisted Sprout's old screeches and growls are from Left 4 Dead's Hunter special infected, being its "warn" sound and "hunter_stalk_04" respectively. Interestingly, the "warn" sound in question appears to be a blend of "hunter_warn_16" into "hunter_warn_14". The old sounds have been replaced as of v0.11.1 due to Qwelver wanting to be safe with copyright.
